Customer Reviews

they are amazing!

We were only there for 42 hours before mom passed away. She was the first person to stay there to just stay for hospice without being a regular resident, and they were super awesome. Brett is beyond compassionate and Kristy was extremely kind and helpful.

Feels like home!

We were very impressed from the moment we walked in the front door. Beautifully decorated with a warm & homey country atmosphere. The staff are very friendly and welcoming. It is one level and not as large and imposing as some of the newer assisted living places. Mom seems to be adjusting well to her new home, and we couldn't be happier!

Friendly staff...but the residents seem uninvolved.

centennial and a place for Mom

very good, knowledge of the system was great

Pleased with the care

Overall I think they do a great job here, my loved one seems very happy. The staff are all very friendly, and keep the place looking nice and clean. One thing I like in particular about this location is the layout. It's set up in a pod system where certain areas have certain caregivers, which means the residents get more individual attention and can be supervised more closely.

Very Pleased

In looking for a Memory Care Unit for my Dad, we decided on Centennial House. The facilities are very comfortable, beautiful and clean. The staff is energetic, friendly, knowledgable and professional. My Dad is happy and doing much better in just a few days. Thanks!

Went to see this on Friday and was very pleased. They are sending a person out to evaluation mom on Tuesday. We are hoping they can take her.

This community is very welcoming and friendly. All employees are smiling and always there to provide assistance. My mother is enjoying being there.

Different kind of layout in pods. Did not like that there was no appliances in the rooms and they were very small rooms. This is more like a nursing home that requires all meals to be prepared by staff. People were very friendly and the layout was nice, but not what I would consider assisted living.

Was pleased with the tour. Only one year tie in. Can apply for Elder Care at that time. Do not turn people away if they run out of money.

First place I visited. Very nice, rooms a ittle small but would do. Window air conditioner in room a negative. Smaller homey feeling a plus. Memory care and EW available. This place is the best choice for my mother.

From the Community

The Centennial House at Apple Valley is a small, personal assisted living option for older adults. It’s two one-story buildings are connected by an all-weather hallway and a sun-drenched gazebo. The gazebo overlooks a spectacular pond frequented six month out of the year by water fowl. The residents spend many happy hours enjoying and watching the invigorating antics of the wild life.

Everyone at Centennial House receives the basic services of meals, housekeeping, social activities and laundry. If there is a need of dispensing medication, help with ambulation, or any additional personal needs, the services are available. Centennial House is affordable because you pay only for the services that you need. We have a simple point system for determining the charges for your care.

Two programs that distinguish Centennial House from other area assisted living sites include the RISE Program and our Enhanced Care Cottage. The RISE program focuses on physical muscle strengthening through group activities and one-on-one training sessions. Our on-site Physical Therapy Assistant is able to identify individual training requirements and set personal goals with residents who participate in the program. Our Enhanced Care Cottage provides higher staffing ratios, more individualized care and specialized programs.

Using our eight years of experience and knowledge, Centennial House has developed an intimate setting with small neighborhoods and home style dining. Our residence feels like home where you get to meet your neighbors and they soon become like family.
