Falls at Brookdale Yorktowne

I chose Brookdale at Yorktowne as an assisted living facility for my mom because it is a beautiful facility and reasonably priced when compared to others in the area. My mom has a history of multiple falls at home in recent months along with increasing confusion and a progressive deterioration in her ability to care for herself. Because I live out of state, I had no first hand knowledge of any of the assisted living facilities in my mom’s area. After doing some research and visiting several facilities, I chose Brookdale at Yorktowne because they assured me that they would be able to take good care of my mother. I told them I was concerned about her falling again, also made them aware that she is confused at times and would need to be checked on to be sure she was safe and not on the floor in her room. Again, they assured me that they would be able to check in on her at least every 2 hours and felt they could provide her a safe environment.
On her 2nd day there, she fell in the bathroom when the aide left her there alone on the toilet—even though I had stressed to the admitting nurse and the floor nurse that she was most susceptible to a fall while in the bathroom because she would stand up to pull her adult diaper up and lose her balance. Apparently this information was not passed on to the aide taking care of her. This fall resulted in her requiring stitches for the gash in her forehead. She then fell a second, then a third time and we were told that we needed to hire around the clock private aides to sit with her to prevent her falling again. These aides were hired at the going rate of $17.00/hour—this on top of the $3200.00 monthly charge to live at Brookdale. We quickly realized that Brookdale was not going to work out for my mom because the cost was now prohibitive. We did find another facility where she is now getting the attention she needs—0 falls in the more than 2 months she’s been there compared to 3 falls within a few days of each other at Brookdale. I’m sure Brookdale is fine for someone who is more oriented than my mom and who is steady on their feet, but for someone who is confused and cannot walk without assistance, do not believe them when they tell you they can provide that level of care—they cannot. Look elsewhere.

Really an Impressive Place

My Uncle is very happy at Brookdale Yorktown he is in the assisted living area and it is very nice, clean and all the staff are friendly and caring. I am quite impressed with the staff myself as they even remember my name when I come to visit my Uncle.
My Uncle likes going to the dining room to eat and he likes to socialize with the other residents while he is there. He likes what they offer on the menu except for the fish but they have lots for him to pick from when they do serve fish.
They keep the residents busy with all the nice activities they offer like Friday has Happy Hour with a
DJ, entertainment sometimes there is a speaker even wine if allowed.
Saturday's there is an Ice cream social and they have an active calendar so the residents know what to expect.
They also offer transportation to doctor appointments, shopping, grocery stores all within a certain time frame, otherwise, special arrangments can be made a head of time.
I would definitely recommend Brookdale Yorktown to my friends and family.

Care for a friend

I like the care of this community. The staff are caring and I like that they keep me informed on any changes/ We like that the community is clean with a nice enviroment. I like the menu and knowing that he likes the variety. They do offer a lot of activites for the residents. We would recommend this community for the care and value.

Very Nice Facility and Pleasant Environment

This is a fine facility and my mother enjoys living there. She wishes there were more active people there or more activities like flower arranging or crafts. She also wishes the exercise classes were more strenuous. Admittedly she is much more active than your typical 94 year old so the program is probably suitable for the majority of residents. I love the staff. They are great and everyone has made Mom feel at home.
One suggestion: They need a formal orientation program for residents and their families on the first day of residence so we know how the program works on a daily basis. No one really discussed sign in and out, what happens if a resident doesn't return to the building, what is the red "door hanger" inside the door for, when and how often is trash picked up, when is laundry day, how do you handle services like phone line installation, will family members be called if Mom doesn't eat well enough or is sick, etc.
Highly recommended!

A Beautiful Community

So far it has been a pretty good experience for my mother with her move to this community. It is still early in the process, but all is well so far. She is still taking some to adjust to the routine around the community. Overall I know she is safe, and they have her best interest in mind. It is a beautiful community, and I would recommend it.

Care of a friend

My friend is at this community. The community was not well kept and as clean as is should of been. The staff members were fair, but a few went above and beyond. The food needed improvement. They did offer him activities but he did not want to participate. I have moved my friend closer to my home and he is doing very well. I would suggest families tour this community for the care of a loved one.


Nice tour of this community, and everything went very well. Everyone was very nice and kind. From what I saw the community appeared to be well maintained. It was a lovely experience.

Brookdale Yorktowne is an excellent Nursing facility. The Staff are very helpful and a pleasure to work with!!

The Management Team are always available and extremely helpful. My only concern has been with the Pharmaceutical
system's lack of flexibility.

We would definitely recommend Brookdale Yorktowne.

Friendly place, with some improvements to be made

I think Brookdale Yorktowne is a nice place, in terms of the facility itself and the attitudes of the staff. Whenever I visit it is always very clean, and the staff members are friendly and pleasant. However, they recently went through a change of ownership which has caused some difficulties as everyone gets used to the new system. It can be a little difficult to get in touch with specific people. But overall it is a nice place.

Last breath of a loved obe

I am writing this with a heavy heart as my grandmother is laying in my spare room about to pass away anytime now.

On Jan 8 2016 Fri night my grandmother took a fall in the memory care facility at Brookdale Yorktown. The facility called an ambulance and she was rushed to the ER. At 4am Sat morning I got a call from Hospice wanting to know if I knew I had a loved one in the ER as they were waiting for a family member to show up. NO I said Brookdale never notified me. My grandmother took a fall so hard she fractured her C2 vertebra and has a hematoma on the back of her head. Same vertebra that took Christopher Reeves life.

Sat morning the 9th I called Brookdale Yorktown to speak with a person in charge. NOT 1 was there. Trudy,Shiela,Carol (nurse), The Director, Kristy. Not even a receptionist.
I told the fella who did answer the phone they had 15 minutes to get someone of importance of that facility to call me. 3 minutes later Trudy called.

Grandma was transferred back to the facility that afternoon with 24/7 Hospice critical care by her bedside. Brookdale Yorktown is suppose to evaluate a patient to see if they even can come back. But since nobody was in charge at the facility -nothing.

Monday the 11th 2016 I had to move my grandmother home as Brookdale Yorktown did not have the care she needs now after her fall. Still Hospice by her bedside 27/7.
That evening after getting grandma situated in her hospital bed hospice delivered here at home. We notice a month old bruise the size of Texas on her left upper thigh. It is so big it is from her hip to her knee. (Took pic). Looks like she had fallen a month ago and yet still no call. My grandmother would have had to get help up if she had fallen.

Oh by the way the fall to her C2 - was a site unseen fall according to Brookdale Yorktown. Nobody will tell us where,how,who found her NOTHING !

To make this short this facility is running on bare minimum staff who are over worked and under paid. 3 weeks ago there director who was in charge walked out on them with no notice. He slipped out the back door.

My grandmother was placed at this facility in Aug 2015 for Alz. They talked a good talk when we got her in there. Then we found out even at level one care witch we paid for we were cleaning her room changing her sheets made sure she was taking care of. Cleaning poop off the walls from my grandmother's roommate Thelma.

My grandmother is now bleeding in her brain from this fall. And she fell breaking her right hip in July 2015 here as well. PLEASE think twice about this facility.

PS they were showing my grandmother's spot to other people while she was lying in bed with Hospice by herself and family that Monday hours before she left.

Thank for reading ...A father is coming this afternoon to read the last rites to my grandmother. Don't let this place make it your families last rites...



It could be a very nice place, but it is beginning to look run down. Many of the people that work there are very nice. Unfortunately, we experienced outrageous over billing on everything from prescriptions that are not needed to the basic and personal service rate (more than 700) dollars a month for six months and counting. Even worse, it has taken what I consider to be an enormous amount of time and effort to get them to correct the errors. It rises to the level that I wonder if it is intentional. It does not seem possible that such flagrant errors can occur for months on end even after it is brought to their attention and they agree that errors have been made. The local people blame corporate and nothing is corrected on the monthly bills or overdue balaence notices. It created a tremendous hassle with long term care insurance delaying payments and causing confusion.

The carpet is old and nasty. The building has green mold looking stuff growing on the siding. I should have considered another alternative before getting stuck there after paying thousands in an unrefundable community fee that they only tell you about at the last minute. I feel even worse for the employees than I do for the residents and I feel very sorry for the residents who are incapable of dealing with these issues. If you think you might need to get them to deal with routine issues in an effective manner, you may want to consider other alternatives. I have spent what I consider an outrageous amount of time trying to straighten out issues with my father's billing along with other issues that should have been handled with one phone call. I am beyond dissappointed, frustrated and completely dissatisfied.

Nice place, overall

We're very pleased with how things have turned out with Brookdale Yorktowne. The staff are friendly and helpful, and they're taking excellent care of my loved one!

It was not a good first impression.

When I visited Emeritus, it was kind of crazy. They did not have any open rooms to show us. One of the woman residents fell while we were visiting. I did not get to see very much of the property as we just stayed in the interview room the whole time. It was not a good first impression.

Very friendly staff!

We've been very pleased with how things have turned out with Emeritus at Port Orange! The staff are all very friendly, and they do a great job utilizing the people that they have! I really like that instead of having a normal and then a weekend staff they spread everyone's schedules out so there is equal coverage for every day of the week. We'd recommend them to others looking into senior care in the area!

Very nice place!

Things have gone well at Emeritus at Port Orange so far. We toured quite a few communities during our search, and this one was our absolute favorite! The staff all seem very friendly, and although it was a big adjustment my loved one seems to be settling in well. The facility itself is clean and well kept, they've got a great variety of activities, and the food and dining service are fantastic!

This has been a good community for us. The staff has been friendly to us. We haven’t had any trouble finding parking. The food has been okay for us too. We’re happy with the level of activities that are offered at this community.

They have a really a really great activities department. It’s a friendly feeling when you walk in. The staff makes you feel welcome. It’s an attractive community. The food is good. There has been enough parking.

It was nice and bright and sunny...

It was nice and bright and sunny, but they only had a refridgerator and a sink so it wasn't as roomy as the place we chose, it was more like a studio. It was more expensive, and the rooms were really small.

It's OK

I had them in there for a brief two weeks and it wasn't what we were looking for so we moved them out. I wouldn't say that it's a terrible place, it's nice enough, but the communication wast as good as I was hoping.

Good Activities

The value is hard for me to answer because I am not familiar with any other place. The activities are really good. My sister-in-law is in memory care, and she has her own room. With the memory care, they have them come out and set the tables, they do singing, and they watch TV together. They also go out with the assisted living people and listen to performers that come in.

I really liked the faciliti...

I really liked the facilities and I felt Mom and Dad would be happy there but I don't think they can swing the monthly fee's. It's too bad, that place is awesome.

Friendly aides and nurses

After a brief stay in another facility my loved ones lucked out and found a perfect apartment here at Emeritus at Port Orange. I would say for an overall experience with Emeritus it's been very good. The aides/care givers are very nice here. They recognize the residents and know their names and are good about distributing their medications. I would say that the meals here are also very good; the previous place had less variety and selection for the meals which they were not happy about.
